United Healthcare Possible lapse?

So yesterday I received an email from UHC saying my claim has been processed. It was for a claim June 29th-July 2nd of 2021. Which had already been processed and paid months ago. I paid for my copays, cool. I thought we were good. They’re saying the premium hadn’t been paid for that week and payment needed to be paid in grace period? (In the denial reasoning)

This is the first I’m hearing about this. When I called UHC the gal on the phone kept saying oh, don’t worry about it. I see you’ve had coverage all along blah blah probably an error.

Now, I did have a leave of absence for work during this time and my employer sent me a bill that I recently paid for unpaid benefits for like one cycle before paid leave kicked in I assume. Is this what they’re referring to? Is there any recourse? UHC has never mailed me anything, let alone saying I owed them anything or to “pay in the grace window.” Now I’m stressing out I’m going to owe the hospital over $30k. It was for an outpatient surgery that went wrong and I ended up in the hospital for days.
