Will need outpatient surgery & trying to select a healthcare plan

Hi all, I’m new here but trying to figure things out. I’m currently uninsured and experiencing my second ectopic pregnancy (when the fetus grows outside the uterus).

I was treated with drugs in the hospital (never admitted) and I’m sure we’ll have a hefty bill but we’ll deal with that later.

Since this is my second ectopic pregnancy in the same Fallopian tube it’s being recommended I have surgery to remove the tube which I’m totally onboard with. I can get the surgery at anytime (as long as my tube doesn’t burst before I’m “cleared” from the drug treatment) but naturally I’d like to get it over with.

I have a bit of time to select a healthcare plan through Obamacare (luckily we have a move to a different state coming up triggering qualifying life event).

Here’s the details of the procedure I’ll need:

Outpatient surgery – single Salpingectomy Surgeon (likely ob/gyn) General Anesthesia Pain medication Consultation, follow up, etc.

And here’s the info I’m hoping to find out:

How do I know how the surgery is going to be billed? Elective, preventative, etc? Is that determined by my doctor or by insurance?

My understanding is Obamacare plans just cover preexisting conditions (this would be considered preexisting). Is that correct?

What specifically do I need to look for when selecting a plan? Initially I was just looking at low/no deductible, but then I saw the little line item stuck in there for “outpatient facility” and realized there’s going to be more to it. Is there anything else I should really be focused on when comparing plans?
