Charged while on COBRA

My insurance is putting me on the hook for over a grand for an injection. I require this shot once a month. In January I had to use COBRA to cover a few-month gap. I elected to maintain the same level of previous coverage. These shots require prior authorization that I am not a part of – the nurses do this directly with insurance. If they do not get prior authorization, I do not get the injection. Needless to say, I received the shot for January. Now in March, I receive a bill claiming prior authorization was not obtained for January, and therefore insurance claims it as out-of-network, leaving me responsible for 1k co-payment for that month. If I had known, I wouldn’t have received the injection. Then again, I need the shot to function. COBRA is supposed to prevent this situation. Do I have recourse?
