Contract Dispute=Out of Network

We have one hospital in our county. It’s where my wife gave birth to our son and it was covered in network then by United. The hospital was in-network in January when she renewed her insurance coverage with United. In June of last year her gynecologist discovered blood in her stool and given the fact that her mother was actively dying from colon cancer at the time he ordered a colonoscopy. The doctor was in network And only operates in this one hospital. We never received any notification, but apparently this hospital and United had a contract dispute and the hospital was out of network for a grand total of six weeks. Colonoscopy was squarely in the middle of this time period and now United is refusing to cover the procedure. We are working through the appeals process, but has anyone else had something similar happen? Good advice on how to resolve it? Is there an obligation on the insurance carrier to let you know if the only hospital in the county gets dropped from the policy you signed in January?
