ISO Medical and Rx coverage in Kentucky

Medical, Rx, Dental, Vision are needed for two adults, 35M and 33F. $500 or less per month. Ideally no/low deductible, 80/20 coinsurance, covered preventative care, $20-30 copay for medical visits, and generic Rx for =<$15 each. (Am I searching for a unicorn?)

I am currently employed but will be quitting my job to stay home long term and will no longer have insurance through my employer in Kentucky. The only options that seem to be available are four iterations of an HMO through Anthem, listed below. I would prefer a PPO in case I need to use other doctors who aren’t in network, are there any other options for Kentucky? I am assuming they aren’t showing up in an online search because a qualifying event is needed to enroll, but I will of course have a qualifying event if I leave my employer. My spouse is self-employed so does not have insurance.
