Confused about deductible and what counts as preventative care

Newly graduated young adult. On my own health plan.

It’s Anthem High Deductible. I made an appointment back in January for a March physical. I’m in good health.

The person scheduling me was…rather unhelpful and all I asked was if they cover Anthem. When I got to the doctors office, they did accept my insurance but once in the exam room, I was told this was a “establish relationship” appointment and I won’t be examined. Okay. Not a big deal but not what I expected.

I checked my account and Anthem says the amount billed was $376

Under Plan Discount, it subtracted $113.78. The amount allowed by the plan is $262. It says I pay $262. I guess I mistakenly thought the visit would be completely covered? It states $262 is applied to my deductible, so I don’t exactly know where the deductible thing plays if the provider was “in network?” I had no idea of this πŸ˜–

The medical code is 99203 “Outpatient Visit, New”

Can someone break this down?
