On Kaiser insurance with a major specialist surgery booked, got job offer that only offers Anthem, advice?

Hello I’m a young recent college undergraduate and have been on California Medi-Cal (medicaid) via Kaiser throughout college. I started the process of trying to get a very specific surgery with a Kaiser specialist approx. 3 years ago. I finally got a date for surgery in a few months with this specific specialist. However, as a new grad, I got offered a job that I would like to accept so I can gain experience and start my career. However they told me that they do not offer Kaiser plans and only offer Anthem. I would hate to deny this job, but I also have been waiting years to get this specific surgery for a condition I’ve suffered from.

What are my options and what are specific questions I should ask the employer’s HR regarding this? Right now I see my options as this:

Deny Anthem from the employer, pay out of pocket for Kaiser. This might be rough financially since the pay isn’t the best and I would be having to take un-paid time off for 3 weeks after surgery (employer said this is okay). Advice on this option appreciated.

Accept Anthem, cancel surgery and start over by finding another doctor and waiting.. Maybe not ideal because unsure how long I would be with this company, and I feel that would make years of waiting and many appointments feel wasted. Also I am actively in pain.

Deny the job, continue my part time work and focus on career finding after my surgery is complete and I have healed (suggested by friends but seems irresponsible? I would hate to put my career on hold even though this company is far from my dream job)

Are there any other options I am possibly missing? What can I ask HR? How do I approach this? Thank you.
