My pharmacy benifits are different depending on what documents you look at

On one page, it says a tier 3 drug is a $55 copay, but on 2+ different pages is says a tier 3 is a 30% coinsurance.

On the formulary it says tier 3 30% coinsurance for the specific drug in question( a $90 retail inhaler) and yet they’re still charging $55 dollars. Why would they have such conflicting info, and is there a way to get them to stick to the 30% over the $55?
