BCBS Card Clarifications

I have BCBS PPO through my job. I regularly have to visit the pharmacy for a prescription that keeps me alive. Each monthly cost is about $250 after they have my card info. I need clarification on what certain things on my card mean, and whether or not my deductible applies.

Is the $250 a “discounted” amount thanks to my insurance? I am not sure what the uninsured amount of the prescription costs, but I do know that it is not a cheap medicine and $250 feels very low.

Card reads IND Deductible N/A in-network and $500 out of network. IND Out-of-pocket is $2500 in both in and out of network. What is IND? Induvidual? I am the only user of the policy FWIW.

Assuming my pharmacy is out-of-network, does this mean I only need to make 2 months worth of pharmacy runs before I reach my deductible? Is it safe to assume that these out of pocket costs each time I pay for my script are going toward my deductible?

I don’t see coinsurance listed on the card, but I do see it say “PPO $25 copay”, does this mean I do not have coinsurance? It also says “RX $15/$25/$35” under that, I assume this is relevant?

If yes to above, is the only way I’d then reach my $2500 out of pocket through a large number of $25 copays?
