USA. What questions/codes/etc should I ask my doctor for and my insurance company for to find out if a test or drug or xray will be cheaper to have insurance cover it vs going to a 3rd party company to do it out of pocket?

Its a real question. In the USA. There are some situations where a third party company (Mark Cuban’s drug company?) or a 3rd party blood testing company will not take insurance but be so much cheaper that you’re better off using them and paying out of pocket.
