I’m on a UHC HDHP. The plan SBC says the following are covered before meeting deductible:
Preventive care, Office visits, Children’s eye exam, and Children’s dental check-up, are covered before you meet your deductible.
My child has had a couple of minor injuries, and both times we’ve schedule appointments with the pediatrician to have them looked at. We received EOB documents for both appointments, and both were billed as “OFFICE VISITS”. Amount billed $200+, amount owed $100+, in network provider.
These are literally billed as “OFFICE VISITS” but they apparently aren’t covered. We’ve paid the bills, but now I’m wondering: per the SBC, what qualifies as an office visit?
