Anesthesia bill won’t send to insurance for processing?

I’m genuinely confused by this one. I had a cyst removal in March. All good. Paid my copay and thought that was it. In may I got a bill for 2k for the use of anesthesia (from us anesthesia partners). Curious if I was responsible for the full portion of my bill, I reached out to my insurance company and they informed me that they never received a bill for processing. No worries, I contacted USAP and gave them my insurance info and thought that was it. It was not it. Now, nearly 5 months later and countless phone calls, voicemails, emails, an escalation to the better business bureau (which didn’t do anything but did get me a response from the escalation desk at USAP) they STILL have not sent my bill to my insurance to be processed. I seriously do not know what to do. I’ve sent my insurance card to them so many times, can they not just send it to my insurance for processing? Am I living in cuckoo land or is this not a basic thing? I’m worried it will go to collections despite my best and repeated efforts. What can I do? Thanks all!
