Anthem made visit MORE expensive??

Hi all!

I had a new patient appointment with a urogynecologist back in September. My EOB said I owed $110 (I had a HDHP).

MyChart with the doctors office said I owed $300+ for the visit. So my next visit, I paid the $110. I thought that they were still waiting on information and that it would be updated. It never was, and the doctors office called me to pay the rest. I explained that I was only supposed to owe $110. They resubmitted the claim.

Anthem now says that I owe $150 for the appointment. The doctors office submitted for CPT code 99203. Anthem wanted my medical records to confirm, and said the claim needed to be submitted with code 99204. I now owe $40 MORE than the original cost.

Anthem said they have only received the claim once and updated the EOB. I have no copy of the original EOB saying I owe $110.

The doctors office said they can’t resubmit the claim because Anthem will reject it as a duplicate. What are the next steps for me when I contact Anthem?

(This is the 3rd billing issue I’ve had with Anthem for 2021. 🥴)
