Balance bill from procedure 16 months ago

Arizona – United Healthcare – high deductible/HSA plan

I’m the primary insured. My wife had a colonoscopy in June 2020. Gastro doc billed his bill and I paid the co-pay soon after so I thought this was all closed out.

Last month (Sept 2021) the anesthesia doc sent a bill for $1100; for date of service June 2020. United HC EOB says he’s entitled to $120, even though he’s out of network.

I called my insurance. They said the EOB is correct and that they would write to the doc. I got a copy of the letter, it basically said “send him a bill for $120”. Today I get the same bill for $1100.

I checked the balance billing legislation in effect in Arizona; it says balance bills can go to arbitration by the State Insurance dept., but only if the bill comes within 12 months of the service. Seems a bit suspect to me that this bill comes >12 months later.

So, what’s the protocol here? Can I just ignore this bill because of the time passed?
