Birth Control Covered but not visit?

Hi there, just looking to clarify whether this bill makes sense before paying it.

So I’ve had a birth control implant for 3 yrs, it was time to get it removed, and I wanted to change to an IUD. Set up an appointment with a new gynecologist as I’ve moved, in the first appointment they were able to remove the implant, recommended I reschedule to get the IUD inserted.

My insurance is a United Healthcare HSA plan with a pretty high deductible, and my copays do not kick in until the deductible is met, but my understanding is that due to the ACA birth control is covered. If I did have copay’s kicking in, my copay would be $65 for a specialist visit.

The bill shows:

Office Outpt New 30 Min $175
Removal Non-Biodegrad $250
United Healthcare Payment -$200.51
United Healthcare Adjust -$49.49
United Healthcare Adjust -$44.84
Balance Due $130.16

Which leaves me utterly confused.

First off, does the ACA not cover the office visit in which you get birth control?

If not, why would that $44.84 adjustment make sense? If they were covering until my copay, it would be more, if they were not covering at all because I haven’t met my deductible (this is my first visit on this plan), it wouldn’t exist right?
