Charged $500 by my health Insurance company b/c hospital did not obtain proper prior authorization

My wife recently had a tubal pregnancy and was rushed to the hospital and into emergency surgery. A month down the road we get a bill for the full amount of the surgery. I contact my insurance company and they said it was because the hospital did not obtain proper prior authorization.

We had them run it back through properly and later down the road we receive a much smaller bill, but now my insurance company has charged us a $500 penalty for not obtaining proper prior authorization.

This is clearly at the fault of the hospital. We are not the ones who obtain prior authorization, especially when it’s a spur of the moment emergency surgery.

I called the hospital without expecting much, but hoping they’d refund $500 as this is their error. They gave me the run around and claim they cannot do so.

What sort of next options do we have here? It seems backwards that the patient should be penalized for a hospital not doing their job. Is there anywhere I can go to file a complaint against the hospital that would actually gain some traction?

Thanks!
