Charged for preventative service to get papers signed

Hello everyone! I’m 19 and obviously don’t have much experience dealing with health insurance problems, so thank you in advance for being patient with me. I feel like I need to include a lot of background for this.

For a quick overview (in case the background is a bit excessive), I had to make an appointment with my doctor to get a form filled out and signed. I am now being charged for ‘preventative services’ because my insurance plan’s benefits do not cover it. I would like to know if getting a form filled out counts as a preventative service and, if not, what steps I need to take to dispute this. Other important things to include are that I am a dependent being covered by my father’s insurance and that my doctor is not in our plan (this was new news to me and definitely getting addressed in the future).

My college has required all new students to submit proof of immunization records in accordance with the New Jersey Department of Health. I had two options: either submit records for each of the required vaccines or submit a form where the dates for each vaccination needs to be filled out and my doctor needs to sign it.

I went to my family doctor in late March and requested my records and 1, was told that they did not have access to my previous vaccine records and 2, was asked to set up an appointment with my doctor in order to have her sign the form. I signed up for the appointment and afterwards had to run to my previous office to sign off permission for my current doctor to have access to my records. This is not the first time I’ve been thrown around between multiple offices to deal with getting paperwork signed, but I’m not sure it’s too relevant here.

A few weeks later on April 19th, I go to the appointment and get my papers signed. The nurse who took me to the patient room did the usual rigmarole before seeing the doctor (weight and blood pressure check, etc.). In hindsight, maybe I should have been insisting on skipping it, but I didn’t know that could have possibly(?) been a factor in getting charged. The talk with the doctor was very quick. I gave her papers from my other office with the dates of the vaccines listed and she filled out my form.

Now, months later, we’ve received a health care summary/explanation of benefits telling us that the visit is not covered. I’m feeling very frustrated with this office and very lost with what steps to take next. Am I able to argue that this was not a preventive service and who should I contact to get this fixed? Thank you!

Edit for extra info for auto mod: I’m 19, my area code is 08205. I believe my father’s income is most relevant to our insurance, I’ll update in the future when I can grab a hold of him.
