Copay vs full payment

Trying to figure out how I need to proceed with this. My wife was referred to an in-network physical therapist. She saw them 12 times over the last few months. Each time they told her she owed her copay of $70 for the appointment, which she paid up front at each visit. Now we just got a bill for her first two appointments and after insurance discount the appointments were $500 each, with the “copay” applied towards it each, but we’re responsible for the balance. She’s a long way to meeting her deductible, so obviously we’ll be paying a ton out of pocket if we have payment for another 10 visits coming at $500 each. How do we know if this is correct? How can they tell us its a copay and then charge us as if its patient services?
