I need to move onto a health insurance plan of my own (for the first time) and have been looking at different plans. I have a question about health care costs that are agreed upon by the insurer and provider.
Say I haven’t met my deductible and have to pay 100% of the cost for a test. When I am billed the cost set by the provider and insurer, will it appear as though my insurer is paying a portion of the total provider cost? Or will it just appear as a cost for a service with no discount?
Also, are there any circumstances when insurance will pay part of the cost for a test (ex. a CT scan) when the deductible has not yet been met (if there is no % coinsurance before deductible indicated on the plan summary)?
