Doc said it was covered, but getting billed for 3400 anyway?

My mom just completed chemo for breast cancer (yayy). This is the 2nd time she has had breast cancer and the doctor said the type of cancer was rare and possibly genetic. She got genetic testing to see if my sister and I were also at risk. Her surgeon said the test would be covered but now she’s getting denied from the insurance company. The letter said they recognize that this is genetic testing for hereditary cancer and “the clinical documentation does not support any details about ongoing management or how the test results will influence management.”

Might not be relevant, but my parents 1st language isn’t English and the lingo they use is already hard to understand for a person who’s 1st language IS English (me)

Thanks in advance for any advice
