Double Billed for the Same Testing

I am currently pregnant and residing in the U.S.

Not too long ago, I had genetic screening done through Natera. I was given two options: pay out of pocket ($349) or bill through my insurance. I decided to pay out of pocket and paid immediately. A few weeks later, I discovered that my insurance was billed and my insurance sent payment from my HRA to Natera. Billing through my insurance resulted in a significantly higher bill.

I contacted Natera numerous times via phone and email and provided proof of both payments (receipt for out-of-pocket payment & EOB for insurance claim). I either receive a run-around or no response. I contacted my insurance and they also tried Natera but with the same result.

I don’t know what to do. I don’t even fully understand the situation. Would this be considered double billing fraud? Should I be appealing the claim (doesn’t seem to fit)?

Any advice would be appreciated.
