Elective procedures out-of-state

I have an issue with my ankle that I believe will probably require surgery to fully fix. I don’t have insurance, but I am eligible for a $359 credit and enrollment through August 25 because of unemployment. I am a Florida resident, but I am fully nomadic and it would be far easier to visit a doctor on the West Coast.

Do plans exist that would cover this nationally? If not, could I sign up in a state that I’m not a resident in but am able to receive care in?

What plan should I choose? Logic would suggest that shinier metals would mean a more comprehensive plan, but some of the $359 silver plans appear to have the same deductible and out-of-pocket costs as shinier metal plans. I have never shopped for health insurance before and don’t understand very much.
