Getting charged coinsurance instead of copay. Anyone have this happen?

I’m supposed to have a $50 copay and no coinsurance for specialists but I’ve noticed the most recent specialists i’ve seen this year, my insurance (uhc) says I owe 0$ copay and instead lists a coinsurance that seems to be 10%. Oddly enough, at one specialist I did pay the copay upfront during the 3 times I saw him, and the EOB didn’t list a coinsurance for the visits, although it didn’t list the copay either, so I’m not sure if it was just a coincidence.

I’m hesitant to investigate because the coinsurances avg much lower than $50 but I’m wondering if this might come back and bite me in the butt at some point…
