Health insurance requesting dependent eligibility documentation and then cancelling coverage despite being eligible?

My mother has health insurance through her union, she pays for child dependents with her dues and pays additional for spousal coverage. Suddenly when my father was in the hospital for COVID, we were told by the hospital my father no longer had health insurance. I checked for myself and found out that I too had my insurance cancelled while my mother’s was still active. Turns out randomly, my mother’s union had requested documentation to verify my parents were still married and that I was still my mother’s daughter. We’ve had this insurance for at least 15+ years. Is it common to request this documentation after years of no issues?

We spent weeks getting documentation together to verify my parents are married (they were married outside the US). During this time, my mother was still paying for my father’s health insurance despite having no coverage (it is automatically taken out of her paycheck). After finally restoring our coverage, the insurance is refusing to pay my father’s bills from having COVID. Is this legally allowed? Aren’t there some type of coverages especially since we are in a pandemic and it is documented my father tested positive for the COVID at the hospital?

For context, we live in NY so I’m not sure if there any additional state memorandums regarding this.

It’s been months fighting this with over $40K in medical bills that the hospital that my mother works at is threatening to send us to collections for. I am just trying to find any solutions as my mother works two full time jobs and works a side job cleaning houses on her days off in addition to any overtime she can get and my father works 60+ hours as well. Breaks my heart that my father feels he cannot retire (67 yrs old) because he got COVID and got stuck with this bill.
