Anybody know the link where I can download a Membership Authorization Form from Healthcare.Gov?
My mom had a stroke and I need to help re-enroll her. BCBS and her long term insurance Mutual Omaha both required me to have her complete this form to make changes and receive information. So I am assuming the Marketplace does as well. I found an Authorized Aepresentative Form for Eligibility Appeals, but not a standard form that allows me to help make choices on her behalf.
I am working on getting durable POA so please no need to bring that up.
