Help

I have been working on fixing my credit, which requires me to locate all past due balances and medical debt in collections. I’ve found all of it (I think) and requested itemized statements from each one. I discovered that in September of 2018 I was billed $2,200 for ER Services and an ambulance ride. At the time, I had empire blue cross blue shield and it was really good coverage. My ER co-pay was supposed to only be $150. Instead, the ER did not ask for my insurance, nor did I ever receive a bill. I didn’t question it bc I assumed my insurance was on file (since it was an ER through the same hospital my PCP was at). When my debt was taken over by collections, I also never received a bill. They have it on file that the statements / letters notifying me were “returned to sender”. This should have been almost fully covered by my insurance, but I never knew that it hadn’t been covered or even sent to my insurance. I should be able to dispute this (seeing as I never received a bill or had any idea that I owed it until this week), but it is a few months past the timely filing limit. Is there anything I can do? I’ve contacted the collection agency, hospital, hospital’s billing and their financial departments, contacted the insurance and have gotten no where. The insurance agent told me to submit a claim online, but since it is past the timely filing limit, the system will not allow me to even submit the claim. Any help / input would be greatly appreciated. This is stressing me out beyond belief and I am trying so hard to get everything taken care of.
