Help…

My fiancé had a medically necessary endoscopy to check for stomach cancer/other conditions that might explain long term symptoms. We went to an in network doctor, in network health center, and had the procedure pre-approved. They just sent us a bill saying that the procedure cost $4400 and that we owe $1680. We have top tier health insurance, a $2000 deductible that I thought we met, and was under the impression that the pre-approved letter meant that it would be covered? Do I have grounds to fight this? My stomach is in knots and my fiancé is so nervous about money…
