Help: First time enrolling for health insurance

I will soon be eligible for health insurance through my new job. I have been having a hard time deciding between the HDP (with HSA) and the PPO plan. The pricing for each plan is $40/month and $200/month. With an income < 60k/year I would prefer the HDP plan. I will attach the two plans so I can get some feedback. unfortunately, the PPO plan seems more logical because I have some shoulder issues that I am planning to take care of ASAP. This would require Physical therapy, MRI’s and of course a doctors visit for a referral to PT. I have gone through the process before when I was on my parents insurance. The only reason im having a hard time choosing is because my job provides $1300 annually to the HSA account tied to the HDP plan. Essentially this lowers the deductible for both plans to around the same, no? Please help me make the right decision and feel free to ask me questions if more info is needed.
