Help understating insurance claim next steps

I had a septoplasty and turbinate reduction last month and my insurance keeps denying the claims from the hospital and ENT. The reason for denial: “We are unable to process this claim without the copies of the medical treatment records including initial exam, any radiological exam results and/or pre-operative photographs.”

My question is, do I have to submit copies of my medical records related to this surgery or does the ENT/hospital need to submit these?
