Hospital paying off entire medical bill? Is this common?

Not sure if this is the appropriate subreddit for a question like this, since I’m a med student with zero knowledge of insurance stuff.

A friend of the family recently stated how the hospital billed them $0 and $0 to their insurance provider too. The overall bill of 200k was paid by the hospital. This family is wealthy, have great insurance, and not on any state aide.

How in the world could this be?

Edit: They initially thought it could be an error. All 3 installments of the bill was 100% covered by the hospital.
