How can I keep insurance?

I started a new job in January. After accepting the job and prior to joining the company I was moved roles/teams. So on my first day I am already in a job I didn’t apply for with a team I do not know and have not met. While I am still employed there it is not going well and both my interest and motivation is extremely low. A few weeks ago I reestarted my job search. I would like to quit prior to having something else lined up, but I recently had a minor health issue that will require a surgery scheduled for the end of this month. I have plenty in savings to pay my bills/living expenses for an extended period of time, but a large medical bill would be a big hit I’d like to avoid. In our employee hand book it mentions if a period of unpaid leave is approved you get to keep your insurance coverage. Then when you return employment is not guarenteed. Which is fine by me since I am looking to leave, it’s a lousy fit.

Is it out of line to discuss an unpaid leave with my manager so they no longer need to pay me for my poor performance and I can keep my healthcare until the surgery is complete?
