How do deductibles work and what is an adjustment?

Sort of confused here – 27 y/o male newly on my own insurance through my employer. I get roughly ~$120 taken out of my paycheck each week for health insurance, yet find myself paying all the time.

I get that things aren’t free, but it doesn’t look like my insurance is covering anything, rather they are just making “discounts” for me in the light of adjustments, while I have to pay the remaining amount out of pocket.

My deductible is $3,000, while my out-of-pocket limit is $6,750.
