How do I deal with these charges?

I broke a bone in December. I have a PPO plan where my insurance covers 50-65% until the max is met.

I went to an urgent care day after and had x-rays. The provider is charging me for using urgent care AND the emergency room and a separate charge for a Radiologist reading. I suspect their justification is the urgent care PA sent x-rays to be read by an ER doc. Considering there’s already a charge for the Radiologist, and that I never set foot in the ER can anyone imagine recourse here?

I saw an Ortho doc who put me in a brace. they charged my insurance $1000 for this piece of DME that I found on Amazon for $90. Insurance is picking up almost 60% but it’s still a sizable chunk of change. if they’ve already billed my insurance, is am I stuck with this?

Do I have options here? Thanks so much for any advice!
