How Do Medical Bills Exceeding Out of Pocket/Deductible

(USA) How can your out out of pocket costs exceed your deductible if you have ‘decent’ insurance? I see reddit posts with people saying they have 6 figure medical bills despite having insurance. I understand how this might happen if you can rack up expenses from your hitting your deductible every year, but how do people get hit with 6 figures of medical bills even with decent coverage insurance? My mom had breast cancer, and I’m sure the treatment costs hundreds of thousands, but we didn’t have to pay anything outside of our $15k annual deductible for the few years, but we pretty much hit it every year already with a family of 5. Sorry if this question comes off as privileged, but I want to get a better understanding of the system before I need to get off my parent’s insurance in a couple of years.
