How does one pay for a transplant!?

So, this is a hypothetical. I was watching a TV show on Netflix were a kid needed a kidney transplant. The lady tells the parents, “After deductible, your out of pocket is 20%, and that’s 87000 dollars”.

I’m like wtf

So I google, and I find out the cost is over a million dollars in average: https://www.statista.com/statistics/808471/organ-transplantation-costs-us/

So, even with a pretty good insurance, the parents still need to pay almost 100K. Now I wonder, is this fiction, or is this every day life in the US? I thought the anual max out of pocket would have stoped that at maybe 20K?
