How Medicare works for the elderly who are wealthy

Not sure if this is the best sub to ask but I’m just a little confused about something that came up today.

I take care of my grandmother she’s 86 and can no longer walk. She lives with my uncle and I come over daily to care for her and get paid for it through a home health care agency which Medicare/Medicaid covers.

Today she was talking to her sister on the phone ( neither of them can hear so the phone is on speaker and I can hear everything) she’s 89 and in a similar situation. She can still get around but it’s getting harder. She requires some assistance and has a home health aid come out about twice a week. She was saying how she had to pay for it though. She was complaining about how the aid got paid time and half for Christmas and was mad that she had to pay for it and argued with the agency about it. But what I’m confused about is why she had to pay for it out of pocket. Now unlike my Grandmom who’s only income is SS my great aunt is loaded. Her husband left her very well off. I don’t know exact amounts but they have always been rich and she will live more than comfortable for the remainder of her years. She mentioned she has income that comes in from “this and that”. I’m guessing maybe a pension from her late husband and maybe some other streams of passive income? Not sure. But shouldn’t Medicare still cover at least a portion of her home care? Now her husband was also a world war II veteran so she probably has military benefits as well. But wouldn’t Medicare still be her primary insurance ? I’m just trying to figure out what type of financial situation would result in her having to pay for that? Does having a high income change things?
