How to get insurance to process out-of-network as in-network?

I’m appealing a claim with UnitedHealthcare through an employer. I read in a book written by a surgeon that insurance companies can process out-of-network providers as if they are in-network when no in-network options exist. Anyone have tips for persuading the insurance to do that?

I sent my insurance a letter explaining that there were no in-network options for my procedure, but no luck. I asked the insurance company what else they want from me, but as usual, they won’t tell me. What other documents or magic words must the insurance want for this particular issue?
