My insurance has no deductible, 100% coverage of all relevant services, but I keep getting letters in the mail titled “explanation of benefits” where they tell me they are just not going to pay for all of it. How do I write an effective appeal? I work hard for this insurance and I’m genuinely insulted by the nickel and diming and the shameless lying. Any advice?
