I feel like something isn’t right with my insurance and medical bill, help?

I pay a co-pay every time I go for an office visit at my GP, it used to be like $25-$30 when I had a different insurance and now it’s like $60 or so. I pay this AT the appointment. But I always end up getting sent a bill for more. I don’t get it. I always make sure I pay the full amount before I go in for another appointment or once I get the bill (don’t wanna cause bad blood with my doctors office).

Anyways, growing up I always thought you just pay your co-pay for the visit and that’s that. I understand getting billed for extra service (like when I needed my ear irrigated) getting bill once insurance adjust but idk. I might need to change GPs. I can’t keep pay $120 for office visits.
