I forgot to send my Incident Questionnaire for my MRI

I know I need to call them, and I will, but it’s Saturday and I am FREAKING OUT and have had a panic attack.

I forgot to send the incident questionnaire to Premera for my MRI. There was no incident, I just needed an MRI, which they approved before I had it. They pay 80% with my plan.

Then at some point they sent a questionnaire to me to return within 45 days. They wanted to know if anyone was at fault. I forgot to send it because I’ve been having a hell of a bad time and can’t seem to keep total track of everything required in adult life, which is a lot.

Now they’ve adjusted the claim to zero coverage and the MRI is $5000. So I located the form and sent it today, which is about 3 weeks late. Has this happened to anyone, and if so will they simply re-adjust after receiving the form?

tbh I was a little annoyed they didn’t just send a follow-up letter or something. What if it got misdelivered, for ex? That happens all the time in my apartment complex, and someone would just throw it away… it’s not like it was tracked and signed for…
