Insurance company refused lab tests

My insurance company covered one of my bloodwork bills done through Quest lab, and not the other (both from the same dr office). When I called to verify they claimed they don’t cover Quest labs (even though coworkers with the same insurance and coverage use Quest regularly). But I just now called my insurance for prior authorization and they confirmed they do cover Quest “when in network”.

So, how is a lab sometimes out of network and sometimes in? And what rights do I have to re-appeal the Quest lab that they didn’t cover previously?
