Insurance not covering lead screening for 1 year old

My 1 year old had his 12 month wellness visit and they did a lead screening. They drew the blood in the office and sent it to Quest for the analysis. Both Quest and the Dr are In-Network with his insurance (UHC, high deductible employer plan, Plan originates from FL, however we work and live in NY).

The insurance paid some but not all the lead screening. Under ACA this is a 100% covered screening. We asked insurance why it was not covered fully and they said it was not billed as preventable.

We then called the Dr and they reached out to Quest which stated it was billed as preventable, and that this is the first time they ever encountered a time that insurance refused to fully cover it.

I’m at a loss on what to do next as both parties are saying it’s the other’s problem. Does anyone have any suggestions on how to fix this?
