My service provider billed me $600, but the insurance company paid nearly $1500 and are basing my coinsurance on the $1500 paid, rather than the $600 bill. When I asked the insurance company why they overpaid, they said they paid the contracted amount. If that was true, I would think the provider would have billed at least the contracted amount. Where do I go from here?
