Insurer counted a single appointment at physical therapy office as three “visits” toward 30-visit limit. Is that legal? NJ insurance/ Pennsylvania providers

My insurer rejected claims that occurred toward the end of the year, saying I exceeded the 30-visit limit, but there were only 27 unique appointments over which those 30 “visits” occurred (one day counted as three and another as two).

It looks like they did count unique services within these days as separate “visits”.

Is that correct/ legal? As a patient it’s not possible to tell what they are counting and what they aren’t. There are descriptions from other doctors that sound much more like PT but didn’t count toward my limit.
