Medicaid Monthly Income Qualification Methodology Problem (Nevada)

My father had a medical emergency in February 2022. At the time, he was uninsured. He operates a sole proprietorship business that is high revenue and high expense. For tax year 202, his annual income qualifies him for Medicaid. For the month of February 2022, revenue for his sole proprietorship was high because he was paid for jobs that were completed in late 2021.

The Medicaid navigator at the State of Nevada is telling me that income is calculated on a realized basis. That is, the amount of money coming into the checking account in February 2022, less the expenses coming into the same checking account in February 2022. Due to this accounting methodology, my father looks really wealthy. However, over the course of a year in 2022, his income is going to probably be at or below Medicaid eligibility amount.

This is problematic because his hospital bill from February is going to be huge. Is there any way I can convince Medicaid or qualify for some kind of special exception to get the February hospital bills covered? We’re trying to negotiate the hospital bill down, but even with a big discount, the bill is huge.
