Missed enrollment with UHC – advice on appeal?

Hi all! I’m hoping to get some advice on a little bit of an unexpected situation.

My dad works for the main transit system in AZ (so government employees) and received a letter in the mail a few months ago that he needed to log in to his company portal and confirm his benefits for 2022. I did it with him and we didn’t make any changes to his benefits, just looked through the info to make sure everything was still the same and confirmed the benefits.

He recently got another notice saying he’s not enrolled for health coverage in 2022 😞 I’m not sure what happened but I’m not able to find any kind of confirmation to prove we didn’t make any changes and just confirmed his enrollment.

We called UHC and they said we can submit an appeal but just looking for any and all advice around that and if there’s anything we can do for a favorable outcome. TIA
