No claim submitted yet b/c Dr is taking 2+ months to sign off on charts??

My husband saw an in-network specialist for a glaucoma screening at the end of March. He paid the $40 copay for the visit that day. Since then, we haven’t seen a claim from the provider come through our insurance, an EOB or a bill. Our insurance has a 90-day window for timely claims from in-network providers.

I called the office earlier this month (May – over a month after his visit) to double-check that they weren’t missing any insurance info. The rep from the billing department said everything on our end was good, that the doctor just hadn’t signed off on the charts yet.

Now we’re at the point where there’s less than 30 days for them to file the claim. My insurance said if they file late, we wouldn’t be held liable for any denied costs. But I’m nervous that they may never file with the insurance and we’ll get a surprise bill in the mail one day.

I feel like it usually only takes 2 weeks at most for a claim/EOB to come through after either of us visits other doctors. Am I wrong in thinking nearly 3 months seems a bit excessive/shady? What should I do?
