NYC – Submitting the NY AOB Form for surprise bill with EOB?

Hi all, I went to a new doctor for the first time in years for a preventative visit. That portion of the visit was bill fined, in network no additional costs. Later I find out that doctor used an out of network lab for all the blood work and I got an EOB saying I may owe ~1500 dollars for this. I called my insurance and explained this was all part of the in network visit and they told me to fill out the NY AOB form for surprise bills and send it in with a copy of the bill. Question is I haven’t received a bill yet, only an EOB stating the lab work was out of network. Should I wait for a physical bill from the lab or just fill out the form and send it with the corresponding EOB? Thanks for any advice.
