Receiving an explanation of benefits from insurance I don’t have?

Hello, I have anthem blue cross blue shield insurance through my job. I went last year for a physical and didn’t see the claim show up under my anthem account. This week I got an explanation of benefits from horizon blue cross blue shield saying that they could not process this claim (my physical) because my insurance wasn’t active at this time.

I don’t have and have never had horizon blue cross blue shield insurance… I thought horizon and anthem were the same thing (both blue cross blue shield) but the rep I spoke to from anthem couldn’t find any of this information in her system.

Does anyone know how this could have happened?

What worries me is the claim form has all my information as well as a subscriber ID and a group number. These are different from my anthem blue cross blue shield member ID and group number. How do I have an ID number if I don’t have this insurance?
