Shopping insurance with major health issues?

So I am moving and need to shop my insurance. My employer has basically made this my problem as I’m the only employee in our small company that requires health insurance (other employees receive military insurance, are part time, are on spouse’s plan, etc). Our health and life agent doesn’t write in the state I’m moving to and doesn’t know anyone else who does, either. I need very comprehensive insurance as I have multiple major health issues and am looking at a likely transplant and probable hospitalizations in the future, in addition to needing to regularly see a variety of specialists. I also take quite a few meds and need a plan that will provide good prescription coverage.

I’m quite lost with all of this and not even sure where to start. Do I look for small business solutions for one employee? Do I go to the marketplace and price there? How do my health issues impact my premium and options for coverage? How do I know that any specific plan is going to be a good choice for someone like me who needs to utilize their insurance regularly vs someone healthy?
